Homeless Healthcare is a not-for-profit medical service providing primary healthcare to people experiencing homelessness across the Perth metropolitan area. Our multidisciplinary team delivers care across a range of settings, including our fixed-site Hub Medical Practice, mobile GP clinics at homeless shelters, women's refuges, residential rehabilitation and accommodation services, a nurse and peer-led Street Health outreach program, an in-reach service at Royal Perth Hospital, and The Respite Centre, a 20-bed medical respite facility supporting people experiencing homelessness who are not sick enough for hospital, but too sick for the streets.

We are seeking a passionate and motivated Youth Worker to join our Youth Street Health Team on a part time basis. Working primarily alongside a nurse, you will provide assertive outreach, engagement, case coordination and practical support to young people experiencing or at risk of homelessness. You will build trusting relationships with young people from diverse and often complex backgrounds, reduce barriers to healthcare and other essential services, and work collaboratively with the multidisciplinary team to improve health, wellbeing and housing outcomes through compassionate, trauma-informed and person-centred care.

About the role

We are looking for an enthusiastic and compassionate Youth Worker to join our growing Youth Street Health Team (3 days a week).

Working day-to-day alongside a Street Health Nurse, you will provide exceptional engagement, support and case coordination to young people aged 15–25 years who are experiencing or at risk of homelessness. As part of Homeless Healthcare's broader multidisciplinary team, you will work collaboratively with GPs, Peer Support Workers and Case Workers, as well as closely with youth sector and community service providers to deliver coordinated, holistic care. This role combines assertive outreach with practical, person-centred support to improve the health, wellbeing and social outcomes of young people with diverse and often highly complex backgrounds, helping them navigate health, housing and social support systems while reducing barriers to care.





This is a part-time position based across outreach locations throughout the Perth metropolitan area.

Pay will be in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010.

Key Responsibilities

- Provide assertive outreach to young people experiencing or at risk of homelessness.

- Build trusting, therapeutic relationships using a trauma-informed and strengths-based approach.

- Undertake comprehensive assessments of young people's psychosocial and practical support needs.

- Work collaboratively with young people to identify their goals and develop person-centred support plans that build on their strengths, priorities and aspirations.

- Support young people to achieve their goals by facilitating access to healthcare, housing, education, employment and community services.

- Advocate on behalf of young people across housing, health and social service systems.

- Work closely with nurses, GPs, Peer Support Workers and the broader multidisciplinary team to provide coordinated, holistic care.

- Collaborate with youth sector and community service providers to ensure integrated, responsive support.

- Participate in multidisciplinary case discussions and care planning.

- Maintain accurate documentation and contribute to service reporting and continuous quality improvement activities.

Essential Criteria

- Diploma or tertiary qualification in Youth Work, Community Services, Social Work or a related discipline (or working towards).

- Understanding of trauma-informed, strengths-based and person-centred practice.

- Knowledge of the youth, homelessness and community service sectors.

- Experience in case support, crisis intervention, safety planning and advocacy.





- Demonstrated ability to engage and build rapport with young people from diverse backgrounds.

- Experience in case support, crisis intervention, safety planning and advocacy.

- Excellent communication, organisational and interpersonal skills.

- Ability to work independently while contributing positively within a multidisciplinary team.

- Current WA Driver's Licence.

- Current National Police Clearance.

- Current Working with Children Check.

- Current First Aid Certificate (or willingness to obtain).

Desirable Criteria

- Experience supporting young people experiencing homelessness, housing instability or other complex social issues.

- Experience working in outreach services.

- Understanding of the health impacts of homelessness and social disadvantage.

- Experience working collaboratively with health professionals.

- Knowledge of alcohol and other drug services and mental health supports.

- Experience supporting Aboriginal and Torres Strait Islander young people and young people from culturally diverse backgrounds.

We offer salary packaging of up to $15,900 plus additional salary packaging of up to $2,650 for meal and entertainment expenses
