About the role

The Returned & Services League of Australia (S.A. State Branch) is seeking a full-time Care Coordinator to join our team in Adelaide, SA. In this crucial role, you will be responsible for coordinating and delivering high-quality care and support services to veterans, serving members of the Australian Defence Force, and their families. Your work will have a direct and meaningful impact on the lives of those who have served our country.

What you'll be doing

- Develop and implement individualised care plans to address the unique needs of each client

- Facilitate connections between clients and trained RSL Support Officers

- Liaise with a range of health and community service providers to coordinate the delivery of tailored support services

- Regularly monitor and review client progress, adjusting care plans as needed

- Maintain detailed case notes and records in accordance with organisational policies and procedures

- Provide emotional support and practical assistance to clients and their families

- Advocate on behalf of clients to ensure they receive the care and services they require

- Participate in team meetings and contribute to the continuous improvement of care processes

What we're looking for

- Relevant qualification in a field such as social work, nursing, or community services

- Experience in a care coordination or case management role, preferably within the community services or healthcare sectors.





- An understanding of the NDIS would be advantageous

- Excellent communication and interpersonal skills, with the ability to build trusted relationships with clients

- Robust problem-solving and decision-making abilities, with a focus on client-centred outcomes

- Proficiency in documenting and reporting on client progress and service delivery
