Hinchen Recruitment Group is seeking a Wellbeing Practitioner to support veterans and their families. You will deliver holistic case management within a qualified governance framework, combining clinical thinking with compassionate, outcome-focused practice.

You will conduct psychosocial assessments, develop individualized care plans, and coordinate care across health, housing, financial and community services for a caseload of 15–20 clients, focusing on sustainable change and risk management.
