This position provides secondary level supports to nurses providing care in the community to client's requiring wound management. This position is primarily based at Parramatta Community Health Centre, with days spent at Auburn Community Health Centre and The Hills Community Health Centre.

It is a community health centre based position requiring travel across sites and into private residences. What you'll be doing As a Clinical Nurse Specialist Grade 2 - Wound Management, you will provide expert clinical advice, education and support to nurses and other health professionals managing clients with acute and chronic wounds.

You will play a key role in delivering specialist assessment, coordinated care planning and evidence-based interventions that optimise healing outcomes and enhance patient care. Conduct comprehensive wound assessments and develop individualised management plans Provide specialist clinical consultation, education and guidance to healthcare professionals Coordinate multidisciplinary care and contribute to complex case reviews Lead quality improvement initiatives and implement advances in wound management practice Mentor and support clinicians while promoting evidence-based care across the service Essential Criteria Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)Qualification: Post Graduate qualification relevant to the position1.

Demonstrated relevant experience with a minimum of three (3) years full time equivalent post registration experience2.



Demonstrated relevant post graduate qualifications in wound management and-or other qualifications and-or experience deemed appropriate by the employer3.
Demonstrated skills and knowledge in case management, including care co-ordination using advanced clinical skills in patient-client assessment and care planning4. Demonstrated ability to provide leadership in the provision and development of clinical practice through the use of evidence based practice5.
Demonstrated ability to communicate effectively with clients, carers and their families and a diverse range of health care providers6. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion7.

Demonstrated ability to utilise information technology within a variety of applications and willingness to undertake training8.
