Position Overview:
We are seeking an experienced and dedicated Social Worker to join our inpatient rehabilitation team. The successful candidate will be responsible for providing psychosocial support and discharge planning for patients within our rehabilitation unit, working collaboratively with our multidisciplinary team to ensure the best possible patient outcomes.

Key Responsibilities:

- Provide comprehensive psychosocial assessments and counseling for patients in an inpatient rehabilitation setting.

- Support patients and their families with emotional, social, and practical challenges related to rehabilitation and recovery.

- Collaborate with healthcare professionals to develop and implement individualized care plans.

- Assist with discharge planning and ensure appropriate community resources and follow-up care are in place.

- Offer crisis intervention and advocate for patient needs, ensuring a person-centred approach to care.





- Contribute to quality improvement initiatives and evidence-based practices within the social work department.

Selection Criteria:

Essential:

- Tertiary qualification as a Social Worker and eligibility for membership with the Australian Association of Social Workers (AASW).

- Demonstrated experience working in an acute healthcare facility.

- Highly developed social work assessment and counseling skills.

- Strong interpersonal, written, and verbal communication skills.

- Ability to manage and prioritize a demanding workload, meet deadlines, and work effectively within the scope of practice.

- A solid understanding of quality processes and a commitment to applying evidence-based practices in social work.

Desirable:

- Previous experience working in a private hospital setting in NSW.

- Previous experience in an inpatient rehabilitation setting.

- Flexible weekday availability to meet the needs of the department.
