Company Description Create Disability Services is an NDIS Registered, independent provider offering leading Support Coordination and Specialist Support Coordination services. The organization operates across the Hunter/Newcastle region and surrounding areas, working with participants to navigate and maximize their NDIS plans. Create Disability Services focuses on person-centered, outcomes-driven support that promotes choice, control, and inclusion. Team members work collaboratively with participants, families, and community providers to build sustainable support networks and improve quality of life.
Role Description This full-time, on-site Support Coordinator role is based in Maitland, NSW. The Support Coordinator will work directly with NDIS participants to understand their goals, interpret NDIS plans, and connect them with appropriate services and supports. Day-to-day tasks include coordinating and monitoring service delivery, resolving issues or barriers, and helping participants build capacity to manage their own supports over time. The role involves regular communication with participants, families, providers, and other stakeholders, maintaining accurate documentation, and ensuring that supports are aligned with NDIS guidelines. The Support Coordinator will also identify risks, troubleshoot problems,



and advocate for participants to ensure their needs are met in a responsive and respectful manner.
QualificationsStrong customer support and communication skills, with the ability to build trust and work collaboratively with participants, families, and service providers.Analytical skills and troubleshooting abilities to interpret NDIS plans, identify gaps or issues, and develop effective solutions.Technical support and basic systems proficiency to use NDIS portals, case management tools, and other digital platforms for record-keeping and reporting.Knowledge of the NDIS, disability sector, and person-centered practice, with a commitment to rights-based and inclusive support.Relevant tertiary qualification in social work, community services, disability, psychology, or a related discipline (or equivalent experience in a similar role).Organizational and time management skills to handle multiple cases, meet deadlines, and maintain accurate documentation.Ability to work in office in Maitland, NSW, including local travel within the Hunter/Newcastle region as required.Current driver’s license, Working With Children Check, and police check, or willingness to obtain these prior to commencement.
