Position's Primary Purpose:

The Physiotherapist will be responsible for providing high-quality, client-centered physiotherapy services to older adults within their own homes. This role focuses on enhancing mobility, independence, safety, and overall quality of life through tailored assessment, treatment, and rehabilitation programs.

Key Responsibilities, Goals and Objectives:

- Conduct comprehensive physiotherapy assessments for clients in their home environment.

- Develop, implement, and evaluate individualized treatment and rehabilitation plans.

- Provide evidence-based therapy to support mobility, strength, balance, pain management, and fall prevention.

- Prescribe and train clients in the safe use of mobility aids and adaptive equipment.

- Work collaboratively with clients, families, carers, and multidisciplinary teams to achieve care goals.

- Monitor and document client progress,



adjusting interventions as required.

- Educate clients and carers on exercises, injury prevention, and self-management strategies.

- Maintain accurate, timely, and confidential clinical records in line with organisational and professional standards.

- Adhere to all relevant legislation, policies, and professional codes of conduct.

- Actively contribute to halo’s clinical governance framework by following policies, reporting risks, and engaging in quality improvement activities.

Position Requirements:

Knowledge

- Bachelor in Physiotherapy (or equivalent).

- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).

- Previous experience in community, aged care, rehabilitation, or home care settings (desirable).

Essential

- Robust clinical assessment and problem-solving skills.

- Excellent interpersonal and communication skills.

- Ability to work independently and manage a mobile caseload effectively.

- Empathy, patience, and a client-centered approach.

- Commitment to continuous professional development and evidence-based practice

- Valid driver’s licence and reliable vehicle.

- NDIS Worker Screening Check.

- CPR/First Aid certification

Desirable

- Knowledge of the Aged Care Quality Standards and/or NDIS Practice standards.

- Falls prevention program delivery experience

Attitude

- Flexible.

- Proactive attitude to problem solving.

- Adaptable to work in a team and/or autonomously.
