This role focuses on empowering patients to manage chronic conditions and improve their overall health outcomes. We can entertain full time or part-time opportunities.

Key Responsibilities

- Prepare Care Plans and Team Care Arrangements

- Conduct health assessments and assist in early identification of chronic disease

- Coach patients on health behaviour change principles

- Collaborate with GPs and a multidisciplinary team to deliver preventative care

- Manage administrative tasks related to Chronic Disease Management appointments

About You





- AHPRA registered Enrolled or Registered Nurse

- Experience in Chronic Disease Management and health assessments

- Strong organisational and communication skills

- A friendly, supportive approach and passion for patient care
