About the Role Use your skills to advocate for older people facing complex geriatric needs who are living in the community. As a Social Worker within Integrated Care Geriatric Services, you will provide comprehensive assessments, support and short-term case management to help clients navigate complex health needs, social challenges and services.

- Assist families and carers with complex care planning and accessing services
- Deliver counselling and therapeutic interventions
- Build partnerships across multidisciplinary teams

About You You are a compassionate Social Worker with sound clinical judgement who is passionate about supporting older people through holistic, person-centred care.

- Social work degree and eligible for AASW full membership
- Manage complex geriatric caseloads with confidence
- Deliver holistic assessments and case management
- Build strong partnerships across health and community services
- Advocate for older people and their wellbeing
