About the role

A unique chance has arisen for a highly skilled Registered Nurse to join a supportive and innovative Community Nursing service in a role that offers autonomy, variety and professional development. This position is a permanent ongoing role working 60 hours per fortnight across day shift, afternoon shift, week-end shifts and after hours on call. As the successful applicant you may pick up additional shifts up to 76hrs per fortnight. Relevant current/recent community nursing or acute nursing skills and experience is essential to ensure comprehensive client assessments, care planning and delivery of consumer centred care can be successfully implemented by the successful candidate.

In this role you will be responsible for the provision of patient focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written. You will need to have current/recent acute or community nursing experience. Experience & knowledge in the areas of wound care, palliative care, HITH, community health care principles (Active Service Model & Social Model of Health) and continence management is highly desirable.

Key responsibilities

- Deliver high-quality, patient-centred care aligned with evidence-based practice, professional standards, and Barwon Health policies.
- Ensure safety, quality, and compliance with national standards, including participation in audits, incident reporting, and continuous improvement initiatives.
- Collaborate effectively within a multidisciplinary team to support coordinated care and contribute to a positive team culture.
- Provide patient-centred care using evidence-based practice, aligned with professional standards and Barwon Health policies.
- Maintain professional and operational standards through ongoing education, accurate documentation, and compliance with OHS and organisational requirements.

What we’re looking for

- Degree level qualification with a recognised educational institution in Nursing
- Registration with AHPRA as a Registered Nurse Division 1
- Current Working with Children Check
- Current Police Check
- Current Victorian Driver’s Licence
- Current and valid NDIS Worker Screening clearance
- Completion of ONLINE - QUM National Standard Medication Chart Training
- Completion of ONLINE - QUM Medication Safety Training
- Information Technology skills including Microsoft Office, Outlook and Patient Management Systems
- Minimum 2 year post graduate nursing experience in an acute hospital setting
- Previous Community Nursing experience or current acute experience
