Care Partner – Aged Care | Cairns Based
Independent Living Support Association (ILSA) is a not-for-profit community organisation providing quality support services to people with disability and older Australians living independently.
ILSA Home Care Services is one of our key service areas, providing unique, personal and quality supports to older Australians receiving Support at Home funding to remain living safely and well in their own homes and communities.
We are seeking a highly experienced, dedicated and values-driven Care Partner to join our Aged Care team in a full-time capacity.
This role would suit someone with strong experience in the aged care sector, ideally with a background in clinical care. The successful applicant will be confident working with older people, families, support workers and other stakeholders to coordinate high-quality, person-centered support.
The successful applicant must also be willing to participate in the on-call rotation, which is currently one week every 9–10 weeks, with appropriate remuneration. Occasional travel to visit clients on the Atherton Tablelands may also be required.
About the role
The Care Partner will work closely with clients, their informal supports and support worker teams to coordinate responsive, flexible and high-quality support.
Key responsibilities include:
- Engaging with clients and their informal supports to understand their needs, preferences and goals;
- Connecting with relevant stakeholders and service providers;
- Linking clients with other services where required to support their needs in a holistic way;
- Establishing effective support worker teams capable of delivering responsive,
flexible and appropriate support;
- Developing and maintaining client service schedules and rosters;
- Supervising the supports being provided to clients;
- Monitoring support communication notes and responding to service delivery issues as they arise;
- Providing on-call support as part of a rotating roster.
About you
We are looking for someone who is:
· Experienced in aged care, home care, community services or a related sector;
· Confident coordinating services for older people living in the community;
· Able to apply a sound understanding of clinical care needs, risk, dignity of risk, and when to seek clinical input or escalation;
· Comfortable working alongside clinical professionals, families and support workers to ensure care is safe, appropriate and responsive;
· A solid communicator who can build respectful relationships with clients, families, workers and stakeholders;
· Organised, responsive and able to manage competing priorities;
· Values-driven, practical and committed to quality service delivery;
· Comfortable mentoring and guiding support workers;
· Willing to participate in the on-call roster.
