About the Role
Frontline Support Services is seeking a dedicated and experienced Case Manager to join our team in Cairns, QLD. This is a subcontracting role (ABN required), offering an opportunity to make a meaningful impact in the lives of children, young people, and families within our community.
As a Case Manager, you will play a key role in coordinating and delivering essential support services to vulnerable individuals and families while working collaboratively within a supportive and professional team environment.
Key Responsibilities
- Conduct comprehensive assessments to identify client needs and develop tailored support plans
- Coordinate a range of support services, including referrals, advocacy, practical assistance, and stakeholder engagement
- Advocate on behalf of clients to ensure appropriate supports and resources are accessed
- Work collaboratively with internal teams and external stakeholders to deliver holistic, client-focused care
- Maintain accurate and detailed case notes and documentation in line with organisational policies and compliance requirements
- Participate in regular supervision, team meetings, and ongoing professional development
- Participate in an afterhours on-call roster shared within the case management team
About You To be successful in this role, you will have:
- A relevant tertiary qualification in Social Work, Psychology, Human Services, or a related field (or currently enrolled)
- Minimum 2 years’ experience in a Case Management or similar role within the community services sector
- Strong interpersonal and communication skills with the ability to build rapport and professional relationships
- Demonstrated ability to manage complex situations and apply critical thinking and problem-solving skills
- Knowledge of child protection legislation, reporting obligations, and trauma-informed practice
- Ability to work collaboratively within a multidisciplinary environment
- Excellent organisational and time management skills with the ability to manage a varied caseload
- Current driver’s licence, valid Blue Card, Yellow Card/NDIS Worker Screening, and ABN
