Job Category: Single Aged Care Assessment System Greatness starts here The Single Aged Care Assessment System (SAS) has been set up by the Health Department to simplify and improve access to aged care services. The SAS includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments. Working as part of the SAS team, and reporting to the SAS Clinical Team Leader, you will be responsible for:
- Undertaking client assessments, reviews and development of support plans to assist clients in achieving their goals and living independently in their own homes and community.
- Assessors will be expected to conduct assessments across all Northwestern Melbourne LGAs.
- Participating in clinical triage and case conferencing.
- Identifying, monitoring and managing risks and leading appropriate escalation.

- Tertiary Qualification in a health‑related discipline directly related to health, aged care or specialist area such as Allied Health, Social Work, or Registered Nurse.
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or other relevant professional association. Fine Print - Applications close 31 July 2026.
