Western Health in Melbourne is seeking a Clinical Pharmacist - Grade 2 to join the Emergency Medicine Pharmacy team. You will deliver clinical pharmacy services in the Emergency Department and Intensive Care Unit, supporting medication safety and evidence-based care for complex patients. Working with a multidisciplinary team, you will perform medication reconciliation, reviews and interventions, plus contribute to antimicrobial stewardship, quality improvement and research activities.
