ANNUAL SALARY RANGE $95,000 to $110,000 OVERVIEW The Community Worker provides person-centred support to people with disability in accordance with the participant's individual goals, NDIS Practice Standards, NDIS Code of Conduct and organisational policies. The role promotes independence, dignity, choice and community participation while ensuring services are delivered safely and respectfully.
RESPONSIBILITIES AND DUTIES
- Promote participant independence, choice, dignity and community inclusion.
- Assist with personal care, daily living activities and community access.
- Support participants to achieve their individual goals and outcomes.
- Administer medication in accordance with organisational policies and procedures.
- Complete accurate case notes, progress reports and other required documentation.
- Monitor participant wellbeing and promptly report incidents, risks or concerns.
- Work collaboratively with participants, families, carers and other stakeholders.
- Maintain a safe, respectful and person-centred support environment.
- Comply with the NDIS Code of Conduct, organisational policies and legislative requirements.
- Participate in mandatory training, team meetings and professional development.
- Perform other duties reasonably required to support participants and organisational operations.
- Able to provide high-quality 1:1 and group support to participants in line with their individual support plans. REQUIRED SKILLS
- Demonstrated 1-2 years’ experience providing person-centred support to people with disability, including supporting individual goals,
independence, choice and community participation.
- Strong understanding of disability support principles, including dignity of risk, inclusion, empowerment and human rights.
- Ability to develop positive and respectful relationships with clients, families, carers, support coordinators and other professionals.
- Excellent communication and interpersonal skills, with the ability to adapt communication styles to meet individual client needs.
- Ability to provide personal care and practical support in accordance with individual support plans and client preferences.
- Ability to identify and manage risks while promoting participant independence.
- Ability to work independently with minimal supervision.
- Strong time management and organisational skills.
- Experience supporting participants with complex behaviours, mental health or high support needs (desirable).
- Collaborate with participants, families, carers, support coordinators, allied health professionals and other stakeholders to achieve participant outcomes.
- Sound computer literacy for electronic documentation. REQUIRED QUALIFICATIONS
- Certificate III or IV in Individual Support (Disability), Community Services, or related qualification.
- Current NSW Driver Licence.
- Current First Aid Certificate and CPR certification.
- Current NDIS Worker Screening Check and Working with Children Check.
- Availability to work flexible hours, including weekends and shifts as required.
