The organisation is supported by a team with more than 20 years of direct community experience, with particular expertise in dementia care. The team partners closely with care recipients and their families to identify needs, set goals, and create pathways that promote independence, community participation, and confidence. BDMSCS provides consistent 24/7 community-based care to people of diverse cultures, supporting individuals at any level of disability or stage of life.
Role Description The House Coordinator and Team Leader is a full-time, on-site role based in Berry Springs, NT, responsible for overseeing the daily operations of a supported house and leading a team of support workers. This role coordinates person-centered care plans, ensures that support is delivered in line with individual needs and goals, and maintains a safe, respectful, and inclusive living setting.
The House
Coordinator and Team Leader organizes staff rosters, supervises and mentors team members, and supports their professional development. Day-to-day tasks include liaising with families, external providers, and stakeholders, managing documentation and incident reporting, and ensuring compliance with organisational policies,
NDIS standards, and relevant regulations. The role also involves promoting client choice and control, facilitating meaningful activities, and supporting individuals to participate in the local community.
Qualifications
Strong leadership, supervision, and team coordination skills, with experience managing support staff in disability or community services settings.
Client-focused care, case management, and care planning skills, including the ability to implement person-centered and goal-oriented support plans.
Excellent communication, interpersonal, and conflict resolution skills, with the ability to build trust and collaborate effectively with clients, families, and stakeholders.
Organisational and administrative skills, including roster management, record keeping, incident reporting, and basic computer literacy (email, documentation systems).
Knowledge of disability support frameworks, safeguarding practices, and relevant legislation or standards (such as NDIS Practice Standards and duty of care obligations).
Previous experience in disability support, aged care, or dementia care, ideally in a house coordinator or team leader capacity.
Relevant qualification in community services, disability, aged care, social work, or a related field (Certificate IV or higher preferred).
Current driver’s license and the ability to travel locally as required, along with required background checks (such as police check, working with vulnerable people check) in line with regulatory requirements.
