UnitingCare Queensland is seeking a Supported Living Facilitator to provide in-home personal care and life-skills support to clients in the community. This role includes travel between client homes and working flexible schedule, typically Monday to Friday, 9am-5pm. You will assess and implement individual Care Plans, encourage independence, and help clients participate in social activities, with support to travel to appointments as needed. A positive attitude and reliability are essential.
