Description
Summary:
The Intake and Engagement Coordinator plays a vital role in the social care sector by facilitating the initial contact and engagement with clients. This part-time contract position based in Ipswich, Queensland, aims to ensure that individuals receive the necessary support and resources tailored to their needs. The coordinator will work closely with clients and service providers to enhance the overall client experience and promote effective service delivery.

Responsibilities:
- Represent the organisation at community events, networking forums and industry seminars to build referral pathways, promote services and support client intake across NDIS, aged care and DVA programs.
- Conduct initial assessments and intake interviews with clients to determine their needs and eligibility for services.
- Provide information and guidance to clients regarding available resources and support options.
- Maintain accurate and confidential client records in compliance with organisational policies.
- Collaborate with social care professionals and community organisations to ensure seamless service delivery.
- Assist in the development and implementation of engagement strategies to improve client outreach.
- Participate in training and professional development opportunities to enhance skills and knowledge in social care.
Qualifications:
- - Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN), Or Diploma In Community Services, Level 4 in Disability.
- - Previous experience as a support coordinator.
- Further education in social care or a related field is preferred.
- Demonstrated experience in client intake, care coordination, service navigation or stakeholder engagement within NDIS, aged care, DVA, community health or a related sector
- Strong knowledge of referral pathways, client eligibility, consent and privacy requirements.
- Strong communication and interpersonal skills to effectively engage with clients and stakeholders.
- Ability to work independently and as part of a team in a fast-paced environment.
- Proficient in using computer systems and software for record-keeping and communication.
