Role Description The Intake Coordinator is a Part-time, Hybrid role based in Adelaide, SA. This position is responsible for managing incoming referrals and inquiries, conducting initial client assessments, and collecting accurate information to support service planning. Day-to-day tasks include coordinating intake appointments, verifying eligibility and funding details, maintaining confidential records, and ensuring a smooth transition into appropriate support services.
The Intake
Coordinator will liaise with clients, families, support coordinators, and clinical staff,
providing clear information about available services and processes. The role also involves updating databases, preparing intake documentation, and contributing to continuous improvement of intake procedures to enhance client experience.
Qualifications
Candidates should possess solid Communication and Customer Service skills to interact effectively with clients, families, and internal teams.
Candidates should possess Case Management skills to coordinate referrals, manage intake workflows, and support appropriate service allocation.
Candidates should possess knowledge of Medical Terminology to accurately interpret clinical information and document client needs.
Relevant experience in healthcare, disability services, community care, or social services intake coordination is highly beneficial.
Strong organizational skills, attention to detail, and proficiency with client management systems and basic office software are required.
Ability to handle sensitive information with discretion and adhere to privacy and compliance standards is essential.
A certificate 3 or diploma in community services, social work, health administration, or a related field is advantageous.
