Are you an experienced Occupational Therapist who is passionate about helping older Australians maintain their independence, safety and quality of life at home?
We are seeking a dedicated Occupational Therapist to join our growing community aged care team, delivering high-quality, person-centred services across Melbourne's Northern and Western metropolitan regions. This is an opportunity to make a meaningful difference through restorative care, reablement, assistive technology and home modification programs. Based on a versatile hybrid work model, you'll split your time between our Brunswick office, working from home, and visiting clients in their homes.
What's on Offer?
- $88,000 salary + 12% Super (pro-rata)
- $8,000 Motor Vehicle Allowance
- 5 weeks annual leave
- Hybrid work model: Brunswick office, work from home & community visits
- Support clients across Melbourne's Northern & Western suburbs
- Weekly training and team meetings
- 4-day week (must work Tuesdays, other days flexible)
Key Responsibilities
- Conduct home-based occupational therapy assessments
- Deliver reablement and restorative care programs
- Recommend assistive technology and home modifications
- Prepare clinical reports and funding documentation
- Work closely with clients, families and the broader care team to achieve positive outcomes.
About You
- Degree in Occupational Therapy
- Current AHPRA registration
- Minimum 3 years' community aged care experience
- Knowledge of Home Care Packages and Support at Home programs
- Current driver's licence and reliable vehicle
- Excellent communication and report-writing skills.
