This is a Fixed Term Full-Time appointment for 76 hours per fortnight with the possibility of extension(s) and/or permanency. The initial period or end date of the Fixed Term vacancy is 30/06/2027.

South Metropolitan Health Service (SMSH) is seeking a Service Manager to lead the team at our Older Adult Health Hub (OAHH) in Cockburn.

Position Description

The Older Adult Health Hubs are a new model of care for the older adult, providing safe and quality care through integrated, community‑based intervention that supports individuals in achieving optimal health. These specialist‑led hubs are part virtual, part physical and deliver services focused on proactive and responsive care, illness and injury prevention, risk prediction and early intervention.

Responsibilities

- Provide strategic and operational leadership for the Older Adult Health Hub.
- Manage and support a multidisciplinary team including Care Navigators, Nursing and Allied Health staff.
- Facilitate integrated care by developing relationships with stakeholders such as medical specialists, nursing, allied health and primary/community care providers.
- Refine and develop the service model to deliver coordinated, person‑centred care to older adults with chronic and complex conditions.
- Oversee day‑to‑day operations, managing physical infrastructure, human resources and financial assets.
- Supervise staff, foster strategic partnerships, identify and address service gaps, and ensure the Hub remains responsive to evolving patient and primary care provider needs.
- Accountable for reporting requirements, including performance metrics, resource utilisation and service outcomes,



to support continuous improvement and transparency.
- Based at the Cockburn Integrated Health and Community Facility; may require travel across the SMHS catchment area.

Eligibility

To be eligible for appointment, applicants must have a valid working visa (for fixed‑term appointments) or be an Australian citizen, permanent resident, or have a visa to live and work indefinitely in Australia (for permanent appointments).
