The HeartScreen is seeking a passionate and experienced Nurse Practitioner to join our growing mobile cardiac diagnostics team across Western Australia. This is an exciting opportunity to work alongside our multidisciplinary clinical team, delivering preventative cardiovascular assessments and cardiac diagnostic services directly into the community.

About The HeartScreen The HeartScreen is Australia's first preventative mobile cardiac diagnostic service, delivering specialist-led cardiac diagnostics directly into GP clinics, aged care facilities, community healthcare settings and corporate workplaces.
Our mobile-first model addresses one of Australia's largest healthcare challenges—delayed access to cardiac diagnostics. By bringing specialist cardiovascular services directly to patients, we facilitate earlier diagnosis, faster treatment pathways and improved health outcomes.
Our services are Medicare-aligned, bulk billed where eligible, and supported by experienced reporting cardiologists and a highly structured clinical operations team.
This isn't a traditional clinic role.
This is preventative cardiology at the frontline of healthcare.
About the Role
As a Nurse Practitioner, you will play a key role in delivering preventative cardiovascular healthcare across a variety of community settings.
Working autonomously, while supported by our cardiologists and experienced clinical and operations teams, you will conduct comprehensive cardiovascular assessments,
identify patients at risk and facilitate timely referral pathways where required.
You'll work within
- GP Clinics
- Specialist Medical Centres
- Aged Care Facilities
- Community Healthcare Services
- Large Corporate Health Screening Programs
You'll have ongoing clinical support from experienced cardiologists and a collaborative multidisciplinary team who are always available when needed.
Key Responsibilities
- Conduct comprehensive cardiovascular risk assessments
- Perform advanced clinical assessments and examinations
- Interpret diagnostic findings and develop evidence-based management plans within your scope of practice
- Collaborate closely with GPs, cardiologists and allied health professionals
- Assist with the coordination of patient care and specialist referrals
- Support the delivery of cardiac diagnostic services within the mobile clinic environment
- Maintain accurate clinical documentation and patient records
- Represent The HeartScreen professionally across all community healthcare settings
About You We're looking for an experienced Nurse Practitioner who enjoys working independently while being part of a highly supportive clinical team.
You are passionate about preventative healthcare, confident in autonomous clinical decision-making, and committed to improving access to cardiovascular care across Australia.
You are approachable, compassionate, adaptable and excited to be part of an organisation making a genuine difference every day.
Essential Requirements
- Endorsed Nurse Practitioner registration with AHPRA
- Minimum 3 years' experience practising as a Nurse Practitioner (or significant advanced practice experience)
- Experience in cardiovascular, chronic disease, emergency, primary care or acute care settings
- Excellent clinical assessment and diagnostic skills
- Strong understanding of cardiovascular disease prevention and management
- Ability to work independently across diverse healthcare environments
- Excellent communication and patient education skills
- Friendly, approachable and patient-focused
- Current Australian Driver's Licence
- Professional Indemnity Insurance
- Australian work rights at the time of application
- Willingness to travel (paid travel provided) to regional and interstate sites when required
Highly Regarded
- Experience in preventative health or cardiovascular care
- Experience working in mobile, outreach or regional healthcare
- Experience within general practice or community healthcare
- Interest in innovative models of healthcare delivery
