Nursing Support Worker Employer: PAK AURORA SERVICES PTY LTD T/A Blossom Community Care Location: Laverton, Victoria Salary: $80,000 per annum + Superannuation Employment Type: Full-time, Permanent (38 hours per week) About the Role Blossom Community Care is a registered NDIS provider committed to delivering high-quality, person-centred disability support services throughout Victoria. We are seeking an experienced and compassionate Nursing Support Worker to provide direct care and clinical support under the supervision and direction of Registered Nurses and other health professionals. This role is responsible for assisting with the implementation of individual care plans, monitoring clients' health and wellbeing, supporting daily living activities, and promoting independence while ensuring all care is delivered in accordance with NDIS Practice Standards, organisational policies, and relevant health care procedures.

Key Responsibilities

- Assist Registered Nurses and other health professionals in implementing individualised care plans and health support programs.
- Provide personal care including bathing, dressing, grooming, toileting, continence management and mobility assistance in accordance with individual care plans.
- Assist clients with rehabilitation programs, therapeutic exercises and mobility activities as directed by supervising health professionals.
- Monitor clients' physical, emotional and behavioural wellbeing, accurately recording and reporting changes in condition to supervising health professionals.
- Measure and record basic clinical observations, including temperature, pulse, respiration, blood pressure and weight, as required.
- Assist clients with medication in accordance with individual care plans,



organisational policies and delegated authority from supervising health professionals.
- Assist with safe transfers, positioning, pressure care, mobility aids and hoists using approved manual handling techniques.
- Support clients with nutrition, hydration and specialised dietary requirements in accordance with their care plans.
- Maintain accurate clinical documentation, progress notes, care records and incident reports in accordance with organisational and legislative requirements.
- Apply infection prevention and control procedures to maintain a safe, hygienic and therapeutic care environment.
- Encourage clients to develop independence, enhance daily living skills and participate in community, social and recreational activities consistent with their individual goals.
- Ensure all services are delivered in accordance with NDIS Practice Standards, Work Health and Safety legislation, organisational policies and relevant healthcare regulations.

Skills and Experience Required
- Certificate III or higher in Aged Care, Individual Support, or equivalent
- Minimum 12 months’ relevant work experience required Australia/Overseas
- Current

First Aid and CPR certificate
- National Police Check and

Working with Children Check (or willingness to obtain)
- Excellent communication and interpersonal skills
- Ability to work independently and in a team
- Empathy, patience, and a strong commitment to quality care

Desirable
- Experience working with elderly clients or people with disabilities
- Valid driver’s licence and access to a vehicle (for community care roles)
- Multilingual abilities a plus
- Ability to work various shifts including weekends and public holidays
