As a Case Manager in our Home Care team,you'll play a key role in partnering with clients to achieve theirgoals and remain living safely and confidently at home.
Working as part of a supportive multidisciplinary team, you'llcollaborate with Clinical Nurses, Allied Health professionals,Support Workers and fellow Case Managers to deliver personalisedcare solutions that promote wellbeing, choice and dignity.
About the role
As a Case Manager, you'll coordinate and manage HomeCare services for a portfolio of clients form South Perth toRockingham.
You will take a holistic wellness and reablement approach tosupport independence, health and quality of life.
This is a permanent position, newly created due to the continuedgrowth of our Home Care services. Working Monday to Friday duringstandard business hours, you'll enjoy the flexibility to balanceclient visits, working from home and regular attendance at our PortKennedy office, with a minimum of one day per week basedonsite.
Reporting to the Operational Support Manager and working as partof a collaborative multidisciplinary team, you'll be supported byexperienced Case Managers, Clinical Nurses, Allied Healthprofessionals and operational teams to deliver outstanding clientoutcomes.
Key responsibilities include
- Building trusted relationships with clients and keysupports to achieve individual goals and outcomes
- Conducting assessments, care planning and regular reviewsusing a wellness and reablement approach
- Monitoring Home Care Package budgets,
service delivery andreferrals in collaboration with clients, colleagues andexternal stakeholders
- Working closely with Clinical Nurses, Allied Healthprofessionals, Support Workers and broader team members tocoordinate secure, high-quality and person-centred care
- Leading, mentoring and supporting a team of SupportWorkersto deliver exceptional care and services to clients
About you
You're someone who genuinely cares about helping older peoplelive independently and confidently. You combine strong casemanagement skills with empathy, organisation and a collaborativeapproach to supporting clients, colleagues and stakeholders.
You'll bring:
- Experience in case management, care coordination or asimilar role
- Strong understanding of person-centred care practices
- Experience managing client budgets and servicedelivery
- Excellent communication and relationship-buildingskills
- Experience leading, supporting and developing SupportWorkers or other frontline teams
- Strong computer skills across Microsoft Office and clientmanagement systems
- A current driver's licence
- The ability to work collaboratively within amultidisciplinary team environment
- You work well with others,
value different professionalperspectives and contribute positively to team culture.
It would be great if you also have
- Knowledge of Home Care Package guidelines
- Understanding of government-funded aged care services
- Experience working with culturally diverse communities
- Experience working alongside Clinical Nurses, Allied Healthprofessionals or other multidisciplinary teams
