About the role
Community Interlink is a partnership between regional hospitals and health services from central and northern Victoria and southern New South Wales.
The alliance assures the quality and availability of locally-governed Support at Home funded care and NDIS Support Coordination.
Community Interlink has an opportunity for a Support at Home Care Manager and NDIS Support Coordinator to Join the Shepparton team.
You will find the Shepparton team asupportive and nurturing group of high achievers who willhelp you to upskill and build your knowledge in both the Aged Care and Disability Support Sectors.
The role provides guidance and support to our vulnerable community members to identify and support the engagement of services that promote independence and support attainment of goals.
This role specifically works with our community members to identify available supports and engage them with the person. Candidates will need to critically evaluate risks to the person and work with them to mitigate identified risks through service engagements and selected supports. The candidate will also ensure that the services that are in place are of a satisfactory standard and liaise with both client and provider to ensure this.
This role will specifically service consumers in and around the Shepparton Region.

- An experienced Aged Care Case Manager or NDIS Support Coordinator
