We’re seeking a Registered or Enrolled Nurse to join our Charlestown Medical Centre, where you’ll support GPs and patients through chronic condition management and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full time opportunities available.

What You’ll Do

Prepare Care Plans and Team Care ArrangementsSupport patients in managing chronic conditions and improving health behavioursConduct health assessments and promote preventative careCollaborate with GPs and multidisciplinary teamsManage administrative tasks related to CCM appointments

About You





AHPRA registered Enrolled or Registered NursePrior experience in Chronic Condition Management preferredStrong communication and organisational skillsFriendly, proactive, and patient-focused approach
