The Salvation Army is seeking a Care Coordinator based in City Of Vincent, Australia, to play an essential role in supporting participants through their withdrawal journey. This part time 0.8 FTE position will enable the team to focus on providing safe and person-centred care by coordinating daily activities across admissions, clinical flow, and discharge planning.
