Company Description Serenity Individualised Support Services provides expert, person-centered care to individuals living with disabilities in both in-home and community settings. The organization focuses on supporting clients to achieve greater independence, participation, and quality of life. Services include community nursing care such as wound care, injection administration, PEG feed management, catheter and complex bowel care, and stoma care. Team members work collaboratively with clients, families, and other health professionals to deliver safe, respectful, and high-quality support tailored to each person’s needs.

Role Description This full-time Graduate Occupational Therapist role is a hybrid position based in Melbourne, VIC, with a mix of client visits in the community and some work-from-home for documentation and care planning. Day-to-day responsibilities include assessing clients’ functional abilities, developing individualized intervention plans, and implementing evidence-based strategies to support independence in daily activities. The role involves working across in-home and social community settings, collaborating with nursing and allied health staff, and contributing to holistic care that may include rehabilitation and support for complex health needs. The Graduate Occupational Therapist will maintain accurate clinical records, participate in case discussions,



liaise with families and caregivers, and engage in ongoing professional development under appropriate supervision.

Qualifications

- Demonstrated knowledge and skills in Occupational Therapy, including assessment, intervention planning, and support for activities of daily living.
- Understanding of Rehabilitation principles and practices, with the ability to contribute to client recovery, functional improvement, and long-term support.
- Strong Communication skills, including clear clinical documentation, collaborative teamwork, and the ability to build respectful relationships with clients, families, and colleagues.
- Awareness of Pediatrics practice or interest in working with children and young people with disabilities, where applicable to service needs.
- Recognized qualification in Occupational Therapy and eligibility for registration or membership with the relevant skilled body.
- Commitment to person-centered care, ethical practice, and culturally safe service delivery.
- Ability to manage time effectively, travel within the Melbourne metropolitan area, and adapt to hybrid work arrangements.
- Previous placement or work experience in disability, community health, or allied health settings is advantageous but not mandatory for graduates.
