About The Role

As a Community Care Manager, you will lead the coordination and delivery of services for older people living in the community, ensuring care is tailored to each client's goals, preferences and individual needs.

You will work closely with clients, families, care workers, allied health professionals and external providers to deliver safe, effective and culturally appropriate services while supporting quality outcomes across the community team.

Key Responsibilities

- Conduct client assessments and develop person-centred care plans
- Coordinate and monitor community support services
- Provide guidance and support to Community Care Workers and field staff
- Support clients using a wellbeing, reablement and restorative care approach
- Manage client budgets and monitor service delivery outcomes




- Identify risks and implement appropriate support strategies
- Maintain accurate documentation and compliance with aged care standards
- Liaise with internal teams, families and external stakeholders
- Contribute to continuous improvement and quality initiatives
- Participate in reporting, team meetings and organisational change initiatives

About You

You are a compassionate and organised qualified with strong case management and communication skills. You thrive in a fast-paced community environment and are passionate about supporting older people to achieve positive outcomes.

Essential

- Relevant qualification in Social Work, Nursing, Allied Health, Mental Health or Disability Services
- Previous case management experience, preferably within aged care or community services
- Demonstrated team coordination experience
- Sound understanding of aged care, rehabilitation and reablement principles
- Strong organisational, communication and documentation skills
- Ability to manage competing priorities and complex client situations
- Current driver's licence
- Current National Police Check (or willingness to obtain)
- Intermediate computer and administration skills

Desirable

- Experience working within the aged care regulatory framework
- Italian language skills or experience working with culturally and linguistically diverse (CALD) communities
