About the role

We’re looking for an experienced and compassionate Specialist Support Coordinator to join our Support Coordination Services team in Palmerston.

This role supports NDIS participants with complex needs, significant risks, or involvement across multiple service systems. You’ll work alongside participants, families, providers and government agencies to reduce barriers, coordinate supports, manage risk and help participants achieve the outcomes that matter most to them.

What you’ll be doing

As a Specialist Support Coordinator, you will:

Provide specialist-level support coordination to participants with complex needs, significant risks or multiple service system involvement

Coordinate supports across disability, health, housing, justice, safeguarding and other community services to ensure participants receive integrated, person-centred support

Respond to crises and changing participant circumstances, coordinating timely and practical solutions that promote safety and wellbeing

Advocate for participants while balancing choice and control with safeguarding responsibilities and effective risk management

Support participants through significant life transitions, including hospital discharge, supported accommodation,



guardianship matters and justice involvement

Build participants’ capacity to understand their NDIS plan, navigate service systems and make informed decisions about their supports

Coordinate multidisciplinary reports and evidence to support NDIS plan reviews and funding outcomes

Develop and maintain strong relationships with participants, families, providers, government agencies and other key stakeholders

Maintain accurate documentation, case notes and reporting in accordance with NDIS Practice Standards and organisational requirements

Manage a busy caseload while meeting quality, compliance and performance expectations

What you’ll bring

You’ll be an experienced professional who enjoys working in complex environments and is passionate about achieving positive outcomes for people with disability.

To be successful in this role, you’ll have:

Extensive experience working in disability, social work, community services or a related human services field, supporting people with complex needs

Strong knowledge of the NDIS, including Support Coordination, participant plans, funding and service delivery

Experience working collaboratively across multiple service systems such as health, housing, justice, guardianship and community services

Demonstrated ability to manage complex situations, solve problems and negotiate positive outcomes for participants

A sound understanding of participant rights, choice and control,



and the ability to balance these with safeguarding responsibilities and risk management

Excellent communication skills, including the ability to prepare professional reports and build trusted relationships with a wide range of stakeholders

Strong organisational skills, with the ability to manage competing priorities while maintaining accurate documentation and meeting required outcomes

High standards of professionalism, confidentiality and culturally protected practice

Confidence using Microsoft Office and electronic client management systems

Current (or ability to obtain) Ochre Card, Criminal History Check, NDIS Worker Screening, First Aid and CPR, NT Driver Licence and influenza vaccination

Qualifications in disability, social work, community services or a related field, together with previous Specialist Support Coordination experience and knowledge of Northern Territory service systems, will be highly regarded.
