Aussie Bridge Care is seeking a compassionate, reliable, and engaging Support Worker/Mentor to support a 17-year-old transgender female participant in the Boronia Heights area.

About the Role
Supports will be provided both in the participant's home and within the community, with a focus on:
- Building confidence and self-esteem
- Encouraging community participation and social inclusion
- Supporting the development of daily living and independent living skills
- Developing positive social connections
- Providing mentoring and emotional support
- Working collaboratively to achieve the participant's individual goals.
