Catholic Healthcare in the Hunter Region seeks a Clinical Aged Care Assessor to join the ACAS team. The role involves local travel across the Hunter region and flexibility with days and times. A clinical background is required (RN, OT, PT or Social Worker with AHPRA/AASW).

You will assess complex needs, refer to appropriate aged care services, and work with clients, families and the multidisciplinary team to support tailored care plans.
