Job responsibilities:

- Perform medication reconciliation activities as directed and communicate discrepancies.
- Work 40 hours per week with 8-hour shifts.
- Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
- Conduct patient intake, focused assessments, and symptom screening aligned with clinic protocols.
