Together Now WA (TNWA) is a rapidly growing not-for-profit charity dedicated to empowering individuals, strengthening families, and building more connected communities. We deliver high-quality disability, youth, family, and community support services that place people at the centre of everything we do.
We're looking for a compassionate, motivated, and organised Client Services Coordinator to join our growing team. Reporting directly to the General Manager (or their delegate in their absence), you'll play a key role in coordinating quality supports that empower participants to achieve their goals while ensuring services are delivered safely, professionally, and in accordance with the NDIS Practice Standards.

As our client's Coordinator, you will manage a diverse caseload of participants while coordinating high-quality, person-centred supports from initial intake and budgeting through to ongoing reviews.
Working collaboratively with clients, families, guardians, support workers, allied health professionals, Support Coordinators, Plan Managers, the NDIA, and community partners, you'll ensure supports are responsive, coordinated, compliant, and focused on achieving meaningful outcomes.
This is a varied and rewarding role that combines participant advocacy, case coordination, funding management, service planning, stakeholder engagement, compliance, and continuous quality improvement.
Key Responsibilities
You will:
- Assist the management of participant intake, assessments, and service commencement.
- Assist the development, implementation, monitoring, and review person-centred support plans.
- Prepare and manage participant Service and funding budgets.
- Coordinate allied health professionals, and external service providers.
- Monitor participant funding utilisation and recommend adjustments where required.
- Prepare participant reports, progress notes, and documentation for NDIS plan reviews.
- Advocate for participant rights, choice, inclusion, and independence.
- Build strong relationships with participants, families, guardians, and external stakeholders.
- Ensure services comply with NDIS legislation, Practice Standards, organisational policies, and quality requirements.
- Work collaboratively with the Client Care team to support effective service scheduling and operational efficiency.
- Contribute to continuous improvement initiatives and the ongoing growth of Together Now WA.
About You
We're looking for someone who is compassionate, professional, and genuinely committed to helping people achieve their goals.
You'll ideally have:
- A Diploma or higher qualification in Community Services, Disability, Social Work, Human Services, Nursing, Psychology, Occupational Therapy, or a related discipline.
- Experience in NDIS case coordination, disability services, community services, or a similar role or a relevant qualification with a genuine passion for building a career in the disability sector.
- Strong understanding of person-centred practice and the NDIS.
- Excellent communication and relationship-building skills.
- Strong organisational, time management, and problem-solving abilities.
- The ability to manage competing priorities while maintaining attention to detail.
- A positive, proactive, and collaborative approach to teamwork.

- Current NDIS Worker Screening Check (or willingness to obtain).
- Working with Children Check (or willingness to obtain).
- National Police Clearance.
- Current First Aid and CPR Certificate (or willingness to obtain).
- Current C Class Driver's Licence.
- Australian working rights.
