About the Role
As our Care Partner, you’ll provide phone‑based clinical case management and support to older people across Greater Sydney, helping them maintain independence, dignity and quality of life while living at home.
The role has a strong clinical focus, supporting clients with complex and evolving health needs, including those requiring higher levels of care and end‑of‑life support.
Working alongside Community Care Workers and a broader multidisciplinary network, you’ll ensure care is coordinated, responsive and aligned with a consumer‑directed care approach.
Key Responsibilities
- Provide phone‑based case management and support to clients under the Commonwealth Home Support Programme (CHSP) and Support at Home (SAH), including completing assessments and developing personalised care plans.
- Use a relationship‑focused approach to identify each person’s goals and abilities.
- Build and maintain strong networks with agencies, health professionals and community services, including ACATs, GPs, social workers and culturally specific organisations.
- Provide person‑centred case management in collaboration with clients, families and carers.

Skills and Experience
- Demonstrated case management experience across Support at Home and Commonwealth Home Support Programme.
- Ability to work in a person‑centred, holistic way to achieve positive outcomes for clients.
- Strong interpersonal and customer service skills, with effective communication across diverse backgrounds.
- Collaborative team player who can balance competing priorities while maintaining self‑care.
- Excellent organisational skills and confidence to represent the service at community events.
- Current driver’s licence essential.
- Relevant qualification in case management, social work or ageing services desirable.
