About Assist Lifestyle: Assist Lifestyle is an NDIS registered NDIS provider based in Birtinya, Queensland, dedicated to empowering individuals with disabilities and mental health challenges. We offer a range of services, including NDIS Core Supports, Psychosocial Recovery Coaching, Support Coordination level 2 and Level 3, and Home and Living supports.

We are currently seeking an experienced Support Coordinator with a strong background in Psychosocial Recovery Coaching to join our team. This is a fantastic opportunity for someone who is passionate about supporting individuals with complex mental health and disability needs to achieve their goals and lead fulfilling lives.

Key Responsibilities:

- Provide tailored, person-centered support coordination and recovery coaching services to NDIS participants.
- Develop, implement, and review NDIS support plans that are focused on achieving participants’ goals.
- Work collaboratively with participants, families, and other service providers to ensure a holistic approach to support.
- Facilitate access to NDIS funded services, including Core Supports, Psychosocial Recovery Coaching, Short-Term Accommodation, and other relevant services.
- Empower and guide participants in navigating the NDIS system, ensuring they have access to the necessary supports and resources.
- Provide psychosocial recovery coaching, focusing on building resilience, independence, and recovery-focused goals for participants living with mental health challenges.
- Monitor and review progress regularly to ensure the best outcomes for participants.
- Advocate for participants and support them in managing any challenges or barriers they may face in accessing services.

Key Requirements:

- Proven experience as a Support Coordinator with a solid understanding of NDIS processes.
- Experience as a Psychosocial Recovery Coach, with a focus on supporting individuals with mental health conditions.




- Excellent knowledge of the NDIS framework and relevant legislation.
- Strong communication and interpersonal skills with the ability to build rapport with participants and stakeholders.
- Ability to work independently and manage a caseload effectively.
- Compassionate and empathetic approach to working with people with complex needs.
- Relevant qualifications (e.g., Cert IV in Disability, Social Work, or related fields).
- Current Working with Children Check and National Police Check.

Desirable:

- Previous experience working with individuals who have complex psychosocial needs.
- Knowledge and understanding of the local community resources available on the Sunshine Coast and surrounding areas.
