About the role

The Clinical Nurse - Advanced Care Planning provides expert clinical knowledge and specialist nursing care within Community Interface Services, Patient Flow Services at the Royal Brisbane and Women's Hospital. The role supports the delivery of high-quality, evidence-based and patient-centred care through the facilitation of advance care planning conversations, education and clinical leadership that promote informed healthcare decision-making and improved end-of-life care outcomes.

The successful candidate will provide advanced nursing expertise in advance care planning, support consumers and families to participate in meaningful care discussions, and contribute to the integration of advance care planning into routine clinical practice. The role also provides guidance and education to nursing and multidisciplinary staff while supporting quality improvement, service development and organisational priorities relating to patient-centred care.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Provide advanced clinical nursing expertise in advance care planning, including assessment, care planning, education and support for patients and their families.
Facilitate and guide advance care planning and end-of-life care conversations that support informed decision-making and person-centred healthcare outcomes.
Provide leadership, education and advice to nursing and multidisciplinary teams regarding advance care planning principles, processes and documentation.




Work collaboratively with patients, families and healthcare professionals to integrate advance care planning into routine clinical care and service delivery.
Contribute to quality improvement, policy implementation, education and service development initiatives that enhance advance care planning outcomes across the organisation.
