Absolute Home Care is seeking an experienced and passionate Case Manager to join our dedicated Support at Home team. This role is ideal for a person who is committed to delivering person-centred care and supporting older Australians to maintain their independence, wellbeing, and quality of life while living safely in their own homes.

As a Case Manager, you will work closely with clients, their families, carers, and a multidisciplinary team to assess needs, develop tailored care plans, coordinate services, manage budgets, and ensure clients receive the right supports to achieve their individual goals.

- Demonstrated ability to communicate effectively with older Australians, families, representatives, service providers, and healthcare professionals.
- Strong negotiation, problem-solving, and relationship management skills.

Collaborative Practice

- Ability to work collaboratively with multidisciplinary teams, allied health professionals, and external service providers to achieve client goals.
- Understanding of aged care regulatory requirements, quality standards, and best practice expectations.

Goal-Oriented & Client-Focused Approach

- Ability to identify client goals, preferences, strengths, and support needs.
- Commitment to delivering consumer-directed care that promotes independence and choice.

Essential Criteria:

- Relevant qualification in Case Management, Social Work, Community Services, Aged Care, Nursing, or a related discipline.
- Demonstrated experience working within the aged care sector, preferably with Home Care Packages or the Support at Home program.
- Strong understanding of the ageing process, associated risks, and the importance of proactive care planning.
- Ability to work independently while contributing effectively within a multidisciplinary team.
- Strong understanding and commitment to person-centred care principles.




- Ability to develop meaningful care plans, conduct home assessments, identify risks, and implement appropriate strategies to support client wellbeing.
- Current Working with Vulnerable People (WWVP) registration, Police Check, and Driver’s Licence.
- Willingness to complete an insolvency check as part of the employment process.

Key Responsibilities:

- Conduct comprehensive client assessments to identify individual goals, needs, preferences, risks, and support requirements.
- Develop, implement, and regularly review consumer-directed, goal-based care plans.
- Coordinate and monitor services to ensure clients receive safe, effective, and appropriate support.
- Engage with allied health professionals, medical providers, community services, and contractors.
- Establish, monitor, and manage client budgets in partnership with clients and/or their representatives.
- Ensure accurate documentation, care records, and service monitoring are maintained.
- Conduct home risk assessments and implement strategies to promote client safety and independence.
- Regularly review client care outcomes and adjust supports as required.
- Support continuous improvement initiatives and contribute to quality audits and compliance requirements.
- Quality & Compliance

The successful candidate will have knowledge of the Aged Care Quality Standards and demonstrate a commitment to maintaining high standards of care, safety, and documentation. The role will contribute to internal reviews, audits, and continuous improvement activities to ensure services delivered by Absolute Home Care meet regulatory requirements.

Job Type: Full-time

Pay: $64,232.00 – $78,000.00 per year
