Part Time / Contract

About your role
As an Aged Care Onsite Pharmacist (ACOP) you will work directly in a residential aged care facility to improve resident medication management and safety. You will collaborate with the healthcare team, residents, and families to optimise medicines, conduct medication reviews, resolve medication-related problems, and provide day-to-day continuity of care.
Key responsibilities of an ACOP
- Medication reviews: Perform regular and personalised medication reviews for residents.
- Medication management: Ensure the safe and appropriate use of all medications, including high-risk drugs, and manage transitions of care.
- Collaborative care: Work as a member of the multidisciplinary team, collaborating with residents, families, GPs, and facility staff to improve health outcomes.
- Issue resolution: Address medication-related problems promptly and provide ongoing support to staff and residents.
- Quality improvement: Support safe use of medicine activities, assist with accreditation,
and conduct whole-of-facility quality reviews.
- Education and training: Provide medication education to care staff and help improve their understanding of medication management.
Must haves
- Aged Care Onsite Pharmacist Credential
- A recognised undergraduate pharmacy degree or equivalent, and unconditional AHPRA registration as a pharmacist in Australia
- Personal/Accident insurance recommended
- Workcover insurance policy if required
- Skilled indemnity insurance policy
- Australian Business Number (ABN)
- Australian citizen or hold permanent residency status in Australia
- Appropriate police clearance and statutory declaration
- Annual flu vaccination (unless medical exemption)
- COVID 19 vaccination (unless medical exemption)
- Working with Children Check and/or NDIS Clearance (if required)
- Basic proficiency in appropriate computing applications
- Excellent clinical skills
- Strong organisational, planning and time management skills
- An interest in relationship building with healthcare professionals and others working in the aged care industry
- Effective communication skills; listening, written and verbal; willing and able to communicate with aged care residents and their relatives as well as colleagues and other healthcare professionals and aged care staff
- Personal laptop
