Job description

The Allied Health Assistant (AHA) reports to the Senior Clinical Manager – Home Care and provides quality support services to the Allied Health team and aged care clients of Rembrandt Living. The role focuses on delivering person-centred, goal-oriented care that promotes independence, mobility, and improved function in daily living.
Key Responsibilities
- Work as an Allied Health Assistant within a multidisciplinary team, delivering in-home support and facilitating exercise groups and individual exercise-based sessions designed by the physiotherapist.
- Support clients in completing prescribed exercise programs that enhance mobility, strength, independence, and overall wellbeing.
- Contribute to and participate in education programs for clients, family members, and staff to improve knowledge of health, mobility, and safe care practices.
- Participate in scheduled client reviews and buddy visits with physiotherapists and occupational therapists to assist with assessments, progress monitoring, and treatment adjustments.
- Ensure allied health equipment is properly maintained, cleaned, and stored, ensuring safety and readiness for client use.
Job requirements
- Strong understanding of the Aged Care Quality and Safety Standards and their application in delivering safe, person-centred care.
- Certificate IV in Allied Health Assistance (Mandatory)
- Knowledge of the Support at Home funding scheme and how it supports service delivery for older Australians.
- Experience working within a multidisciplinary allied health team, collaborating with professionals to achieve positive client outcomes.
- Understanding of the range of services available to aged and disabled individuals, enabling appropriate support and referrals.
- Ability to work effectively both independently and as part of a team, maintaining professionalism and cooperation in the workplace.
- Capability to follow treatment plans developed by allied health professionals to support client care and wellbeing.
- Strong advocacy skills, supporting the rights and wellbeing of vulnerable older people.
- Good understanding of the ageing process and the ability to communicate respectfully and empathetically with older clients.
- Commitment to ongoing professional development and continuous learning.
- Proficient computer skills, including the use of Microsoft Word and Excel.
- Effective conflict resolution and problem-solving abilities.
Job responsibilities
- Follow treatment plans and clinical guidelines developed by physiotherapists and occupational therapists to deliver effective in-home support services to Rembrandt Living clients.
- Provide high-quality, person-centred care, ensuring services are tailored to each client’s individual needs, preferences, and goals.
- Educate clients and their carers on safe manual handling practices and the appropriate use of mobility aids and equipment to promote independence and safety at home.
- Encourage and motivate clients to actively participate in their treatment plans, supporting improved functional outcomes and wellbeing.
- Maintain accurate, timely, and detailed case notes and documentation, ensuring compliance with organisational and professional standards.
