Make a meaningful difference through quality service coordination
Aspiring futures SA is seeking an experienced and highly organised Service Coordinator to coordinate safe, consistent and person-centred NDIS supports for an allocated portfolio of participants.
Reporting to the Service Delivery Team Leader, you will build robust professional relationships with participants, families, guardians, support coordinators, allied health professionals and participant support teams. You will oversee day-to-day service delivery, monitor participant wellbeing and risk, support staff capability and ensure documentation, incidents and service actions are completed within required timeframes.
This is a hands-on coordination role suited to someone who combines strong participant engagement with sound judgement, attention to detail and a genuine commitment to quality disability services.

About the role
As Service Coordinator, you will coordinate the day-to-day delivery of supports across both adult and child participant cohorts.
Your responsibilities will include:
- Building respectful, trusting and professional relationships with participants.
- Understanding participant goals, preferences, communication needs, risks and support requirements.
- Coordinating participant meetings, team meetings, stakeholder meetings and routine service follow-up.
- Monitoring participant health, behaviour, circumstances and experience of supports.
- Reviewing routine incidents, complaints and risks and ensuring required actions are completed. Escalating serious, reportable, repeated or high-risk matters immediately.
- Maintaining accurate and current participant records, support plans, risk information and compliance documentation.
- Visiting participant homes and service locations to monitor participant experience, staff practice, environmental risks and operational issues.
- Providing participant-specific direction, informal coaching and practical guidance to Team Leaders and Disability Support Workers.
- Coordinating participant-specific induction, training and refresher information.
- Supporting staff wellbeing and escalating fitness-for-work, performance or psychosocial safety concerns.
- Maintaining professional communication with families, guardians, clinicians, support coordinators and other service partners.
- Participating in the approved on-call roster and responding appropriately to urgent after-hours matters.
- Supporting participant transitions, audits and onboarding activities under the direction of the Service Delivery Team Leader.
About you
You will have:
- Demonstrated experience coordinating person-centred disability services.
- Experience supporting participants with varied or complex needs.
- The ability to build professional relationships with participants, families, employees, clinicians and service partners.
- Sound knowledge of NDIS safeguarding, incident escalation, participant risk, documentation and service-delivery requirements.
- Experience providing practical guidance and support to Disability Support Workers and participant teams.
- Strong organisation, judgement, problem-solving and time-management skills.
- Excellent written communication and record-keeping skills.
- The ability to identify matters outside your authority and escalate them appropriately.
- Confidence using Microsoft Office and participant, rostering, quality and communication systems.
- The ability to remain professional when responding to emergencies, conflict, distressing incidents or behaviours of concern.
Experience working with children, psychosocial disability, behaviours of concern, restrictive practices, complex health needs, participant transitions or multi-agency supports will be highly regarded.
Essential requirements
Applicants must hold or be willing and eligible to maintain:
- A relevant degree or diploma with appropriate experience, a lesser qualification with substantial relevant experience, or equivalent experience and skills.
- A current South Australian driver’s licence.
- Access to a reliable, comprehensively insured vehicle.
- A current NDIS Worker Screening Check.
- A current Working with Children Check.
- Current First Aid and CPR certification.
- Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
- Required infection-control training.
- Through Their Eyes or equivalent approved child-safe mandatory reporting training.
- Australian working rights and the ability to meet all pre-employment screening requirements.
The successful applicant must also be able to travel between Head Office, participant homes and service locations, participate in the on-call roster and work reasonable additional hours when required.
