Northern Health are seeking applications from motivated and suitably experienced Registered Nurses to join the Older Persons Care Hub.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.

Services are provided through our six main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.

About the Role:

Under the Strengthening Medicare Initiative, Northern Health are piloting an integrated outreach model to deliver rapid care and support to older persons in the community. The aim of the Older Persons Care Hub (OPCH) is to provide non-admitted specialist geriatric assessments and rapid short-term interventions to older people in their homes who are at high-risk of hospital presentations.

The OPCH will serve as a central point for enquiries for older adults (over 65 years or over 50 years for Aboriginal and Torres Strait Islanders) living at home, care navigation and service coordination across Northern Health's catchment. This is a Care Coordination role requiring skills in screening,



triaging and rapid assessment of complex older patients and determining the most appropriate referral pathway, which may range from virtual management to rapid in-home interdisciplinary intervention. Where appropriate, the Care-Coordinator will provide relevant interventions in the home as part of the rapid in-home service the OPCH will provide.

The OPCH Care Coordinator role is currently a 6 day per week service working towards a 7day a week service (Hours of work 8.00am - 4.30pm). The successful incumbent will need to be flexible with their working hours which will include working on weekends and public holidays. This is a 40hour a week position and is fixed term for six weeks.

To be successful in this role you will have:

- Minimum 5 years clinical experience
- Experience in holistic assessment, care coordination or case management-particularly with older populations with frailty and complex needs
- Coaching and motivating experience
- Demonstrated experience that supports older populations with frailty and complex needs.
- Experience in Community or Emergency Nursing is desirable
