About the role
The Pharmacist – Foundation Residency Program provides routine clinical pharmacy services within the Pharmacy Department at the Royal Brisbane and Women's Hospital while participating in the AdPha-accredited Foundation Residency Program. This structured two-year training opportunity supports early-career pharmacists to develop advanced clinical skills, professional capability and expertise within a large tertiary hospital environment.
The successful candidate will deliver evidence-based clinical pharmacy services across a range of practice areas through rotational experiences, supported learning and multidisciplinary collaboration. The role contributes to safe and effective medication management, patient-centred care, education and quality improvement activities while building the knowledge, skills and experience required for future advanced pharmacy practice.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide high-quality clinical pharmacy services including medication review, medicines information, discharge planning, therapeutic drug monitoring and safe medicines management.
- Apply evidence-based practice and professional judgement to support optimal medication use and positive patient outcomes across a variety of clinical settings.
- Work collaboratively with multidisciplinary healthcare teams, patients and carers to contribute to patient-centred care and continuity of care initiatives.
- Participate actively in the AdPha Foundation Residency Program, professional development activities, education programs and clinical rotations.
- Contribute to quality improvement, medication safety, research and service development initiatives that enhance pharmacy practice and healthcare delivery.
