Role description
Boort District Health is seeking compassionate and reliable Home Care Support Workers to deliver high‑quality, person‑centred care to consumers living in the community. This valued role supports consumers to actively participate in their care, maintain their independence, and continue living safely in their own homes.
The position involves travelling between consumers' homes and providing assistance with daily living activities and lifestyle needs, while promoting dignity, choice, and wellbeing.
Key Responsibilities
- Deliver prescribed, goal‑oriented services that support consumers to remain safe and independent
- Provide direct care in accordance with individualised care plans
- Assist consumers with daily living activities and lifestyle requirements using a person‑centred approach
- Observe and monitor consumers' health and wellbeing and report changes as required
- Support consumers, carers, and significant others to build confidence in remaining at home
- Participate in regular care plan reviews in collaboration with consumers, carers, and the coordinator
- Maintain confidentiality, dignity, and respect at all times
- Ensure service delivery aligns with evidence‑based practice, best practice standards, and effective risk management
About you
Essential
- Certificate III in Home and Community Services, Aged Care, Disability or equivalent
- Current First Aid Certificate
- Current Victorian Driver's Licence
- Current National Police Check
- Current Working with Children Check
