About the Role
This role is based in our Mitcham Community Programs Hub, a vibrant and busy environment. You will work both on the floor and within the leadership team, helping clients achieve their goals through tailored support.
Responsibilities
- Deliver one‑on‑one support in the Hub, home and community settings
- Assist clients with personal care, health needs and daily living tasks
- Mentor and coach support staff to improve service quality
- Contribute to the planning and documentation of individualized programs and staff rostering
Qualifications and Experience
- Strong understanding of individualized support and person‑centred practice
- Experience mentoring and coaching staff to improve service delivery
- Excellent communication, teamwork and customer‑service skills
- Confidence using client management systems and maintaining accurate records
- Ability to work independently and manage competing priorities
- Minimum Certificate III in Disability, Individual Support or Community Services (or equivalent)
- Current First Aid Certificate (including CPR) and training in Assist with Medication, Epilepsy & Midazolam, Through their Eyes and Manual Handling
- Current Working with Children Check and NDIS Worker Check
