Condobolin Aboriginal Health Service is seeking a compassionate and motivated Registered or Enrolled Nurse to join our primary health care team.

Working within our GP Clinic, you will play a key role in delivering culturally safe, high-quality care to Aboriginal people, families and the wider community. This is an excellent prospect for a nurse with AMS or General Practice experience who enjoys working closely with GPs, chronic disease management, health checks and preventative care.

About the Role

Reporting to the Clinic Team Leader, the Practice Nurse will support the delivery of primary health care services including:

- Chronic disease management and care coordination

- Aboriginal Health Checks and preventative screening

- Immunisations and vaccine management

- Wound care, ECGs, spirometry and routine clinical procedures

- Health education and patient follow-up

- Recall and reminder systems

- Supporting GPs with clinical procedures and care planning

The role works closely with GPs, Aboriginal Health Practitioners, allied health providers and community programs to improve access and health outcomes for the community.

Essential Criteria

- Current registration with AHPRA as a Registered Nurse or Enrolled Nurse

- Previous experience in General Practice, AMS or community health

- Strong understanding of culturally safe healthcare delivery

- Ability to effectively manage patient bookings, recalls, reminders and electronic clinical records





- Excellent written and verbal communication skills

- Excellent interpersonal skills, with the ability to communicate with and relate to people from all walks of life and with different cultural backgrounds

- Good problem solving and negotiation skills to enable the resolution of difficult or complex issues

- Current knowledge and experience of immunisation schedules, chronic disease management, enhanced primary care item numbers, infection control, CPR and emergency resuscitation techniques

- Demonstrated care plans and health assessment experience

- Triage and wound management

- Experience with accreditation

- Proven experience working well in a team environment

- Excellent organisational skills, with a high level of attention to detail

- Current Class C Driver Licence

Desirable

- Immunisation accreditation or willingness to obtain

- Previous experience in an ACCHO/AMS environment

- Experience using Medical Director and PracSoft

- Understanding of chronic disease management and nKPIs

- Aboriginal and Torres Strait Islander applicants are strongly encouraged to apply
