Join Meli’s Better Futures Barwon team and play a pivotal role in empowering young people as they transition from out-of-home care to independence.
As the Intake and Transitions Consultant, you will lead early engagement and intake processes, ensuring young people are connected to the right supports at the right time. This is a meaningful opportunity to work collaboratively, amplify young people’s voices, and shape pathways that foster resilience, independence, and long-term success.
As an Intake and Transitions Consultant, you will:
- Lead referral, intake, and pre-allocation processes to ensure timely, responsive engagement with young people entering the Better Futures program
- Facilitate early engagement and holistic assessments using a strengths-based, Advantaged Thinking approach to identify individual needs and goals
- Collaborate closely with young people, families, and care teams to co-design transition plans that centre aspirations, independence, and wellbeing
- Support service coordination and allocation by matching young people to Pathways and Development Coaches and strengthening partnerships with DFFH and key stakeholders
Position Details
- Full time, ongoing position
- 1 FTE working 38 hours per week
- $110,379 p/a + Super + Salary Packaging
What We’re Looking For
We value individuals who bring dedication, empathy, and expertise to their roles. To succeed in this position, you will have:
- Tertiary qualification in Social Work, Youth Work, Welfare or a related discipline, or equivalent experience.
- Demonstrated experience working with vulnerable young people and families.
- Strong understanding of the Children,
Youth and Families Act (2005) and other relevant legislation.
- Knowledge of the challenges faced by young people transitioning from statutory care.
- Proven ability to engage and support young people with complex needs and challenging behaviours.
- Experience working from a trauma-informed approach.
- Commitment to child safety and understanding of the Victorian Child Safe Standards.
