Role Description
We are seeking an experienced Onsite Aged Care Pharmacist to work within a local residential aged care facility (200+ bed band), supporting medication safety, quality use of medicines, and clinical governance.
Key Responsibilities
- Conduct onsite clinical pharmacy services aligned with the ACOP model
- Medication reviews, audits, and quality improvement activities
- Clinical support and education for nursing and care staff
- Liaison with GPs and the multidisciplinary team
- AHPRA-registered Pharmacist
- Accredited with an appropriately recognised ACOP course
- Experience or robust interest in aged care
- Excellent communication and organisational skills
