?? The need-to-know…

Rise is seeking the cream of the crop (aka the very best!) Support Workers for our community clients. We have permanent shifts working Monday to Friday either 8am to 1pm or 12pm to 5pm. We are currently seeking applicants happy to travel East Metro, predominantly working in and around Perth Hills/Foothills. Areas include, but are not limited to the following:

Area 1: Kalamunda, Lesmurdie, Gooseberry Hill, High Wycombe, Forrestfield and surrounds.

Area 2: Mundaring, Swan view, Wooroloo, Carmel and surrounds.

Travel is part of this role. On average, client-to-client travel is around 20 minutes. However, at times, extended travel (approx. max of 45 minutes – 1 hour) will be required, and we ask for flexibility when this occurs. Rosters are carefully planned to reduce unnecessary travel.

Day to day, you will follow client support plans, record/report changes, contribute to programme meetings, and assist clients with activities of their daily living whilst achieving their personalised goals. Responsibilities may also include domestic cleaning duties.

?? The need-to-have…

Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:

- Certificate III in individual support (Aged) or equivalent with demonstrated experience post-studies

- First aid certificate (HLTAID009 and HLTAID010) or willingness to obtain

- Comfortability with providing personal care and essential services

- Ability to communicate effectively with clients

- Driving licence with use of a reliable vehicle for work purposes





- Availability to work Monday to Friday either 8am to 1pm or 12pm to 5pm (willingness to pick up additional hours highly regarded!)

Appointments to this position will be subject to a national police clearance and require pre-employment medical assessment. Costs for new checks will be covered and/or reimbursed by Rise.
