About the role
As a Disability Support Worker at Align Care, you will play a crucial role in supporting individuals with diverse needs and abilities to live independent and fulfilling lives. This position offers flexible work arrangements to suit your lifestyle.
Multiple positions available in Goulburn and the surrounding region.
What you'll be doing
Providing personalised and compassionate care to clients, assisting with daily living activities such as personal care, meal preparation, and community participation
Implementing individualised support plans to help clients achieve their goals and maximise their independence
Advocating for clients' rights and ensuring their needs are met with empathy and respect
Contributing to the development and review of support plans in collaboration with clients, their families, and the broader care team
Maintaining accurate and comprehensive records of client progress and interactions
Adhering to all relevant policies, procedures, and legislative requirements to ensure the safety and wellbeing of clients
What we're looking for
Strong communication and interpersonal skills, with the ability to build rapport and provide empathetic care
Demonstrated understanding of person-centred approaches and the ability to tailor support to individual needs
Commitment to promoting the independence, choice, and inclusion of people with disabilities
A valid driver's licence and access to a reliable vehicle (preferred)
Certificate III in Individual Support or equivalent qualification (desirable)
